Broker/ Producer who works under an agency and commissions are paid to the agency.

Principal/ Authorized Officer for Agency when request is to set up a new agency. Log in using the agency principal SSN and name. Then indicate the agency name and Tax ID in Section 3 of the Producer Appointment Data Sheet
