Anthem of Ohio Employer 1094 / 1095 MEWA Forms Update

1094/1095 Forms for SOCA MEWA and OFB groups are available for download via Employer Access.  Please share the attached with your groups, so they are aware of the process.  Additional details below.

•  Anthem is required to file forms 1095-B and 1094-B electronically with the IRS for all fully insured plans that are ACA and Grandmothered.
• 1095-B forms will be mailed to all fully insured members through the end of February.
• Terminated MEWA groups will need to download their forms via the Employer Portal.  Any terminated MEWA groups will have access to the employer portal for up to 24 months to pull these documents themselves. They will not be mailed.   Please note:  You cannot get to Employer Access through producer toolbox for terminated groups. Access to the portal must be direct.
